Youth Medical Release Form

A Medical Release Form is required for all youth to participate in events. These forms are valid for a year. It gives us contact information as well as medical history to better help us serve you. A parent or gaurdian must fill it out and sign it. You can give it to the Youth Director or turn it in to the office at South Norfolk Baptist Church. This form will be added to this page shortly but you can find it on Resources under Church Forms.
